How
to Gain Your Provisional LGV / PCV Driving Licence

Make an appointment for a medical at your own or any other doctor. Remember to take the D4 form (peach in colour)

Complete Form D2 (green in colour)

Send forms D2 and D4 together with your drivers licence to DVLA Swansea
If you need to send your passport, you can take the application to the Post Office; they will check the passport details and give the passport back to you. If you have a digital passport you do not have to send the passport.

Obtain DSA publications “The Official Theory Test for Drivers of Large Vehicles” and “The Official Guide to Hazard Perception” These are available in Book, Video, DVD and CD formats. We carry a large stock of your choice.

When you receive your Drivers Licence with Provisional Entitlement you can Book your Theory Test by dialling:
0300 200 11 22 - have your licence and credit or debit card to hand.
Book an Assessment Drive to determine your course requirements, this lasts for approximately 1 hour, on completion your instructor will recommend the amount of training to reach test standard.

You can train prior to your theory test, but you cannot take a practical test without the theory.

All Courses are Intensive (not hourly lessons) with the DSA Test on the last day of the course.
